Most healthcare organisations have an incident reporting system. Far fewer have a learning system. This course covers the full loop: what should be reported and what should not, how to write a report that is actually usable, how events are triaged and graded, what happens to a report after submission, and how learning is fed back to the people who need it. It also addresses the reasons staff stop reporting — fear, futility and time — and what changes that. Suitable for anyone who submits, receives or acts on incident reports.
