The Deteriorating Patient: Early Warning Scores and Escalation

Spot the patient who is getting worse, hours before the arrest call.

Foundation · 3 study hours · Clinical Deterioration & Acute Care

1. Program overview

Most in-hospital deterioration is preceded by recorded but unacted-upon abnormal observations. This course covers the full recognition chain: accurate observation measurement, early warning score calculation and its limitations, the physiological meaning of each parameter, structured escalation, and the human factors that cause a score to be recorded and then ignored. It includes the patient who scores low but looks wrong, the effect of ceiling-of-treatment decisions, and how to hand over concern in a way that produces a response.

2. Why this program

  • Covers the whole chain from observation to response.
  • Addresses why abnormal scores get ignored.
  • Includes the low-scoring patient who still looks wrong.
  • Structured escalation practised, not just described.

3. Who it is for

Nurse · Physician · Allied Health · Student · Paramedic

4. Program objectives

  • Measure and record physiological observations accurately
  • Calculate an early warning score and state its limitations
  • Explain the physiological significance of each observation parameter
  • Apply structured escalation appropriate to a score and trend
  • Recognise the deteriorating patient whose score remains low
  • Communicate clinical concern so that it produces a response

5. Learning outcomes

  • Calculate an early warning score from given observations
  • Interpret an observation trend rather than a single reading
  • Apply the correct escalation action for a described score and context
  • Identify a deteriorating patient whose score understates the risk
  • Apply a structured communication format to escalate concern
  • Evaluate a described escalation failure for its contributing causes
