Clinical documentation is reviewed by colleagues, coders, auditors, insurers and occasionally courts, and it usually fails all of them for the same reasons: it is incomplete, imprecise or copied. This course covers the principles of a defensible entry, specificity and its effect on coding and reimbursement, the query process between documentation specialists and clinicians, copy-forward and template risk, timeliness and authorship, and how documentation is reviewed retrospectively.
